Bequest

Suggested Wording for Making a Bequest to Advocate Health Care

You may make a bequest to a program or hospital of Advocate Health Care when preparing your will or by adding a codicil to your present will. Bequests may be of cash, securities, real estate, or other property. Bequests of all sizes are welcome, whether they are outright, contingent, or residual. Your bequest should be directed to the Advocate Charitable Foundation. The following language may be useful to your attorney:

Specific Bequest – "I give the Advocate Charitable Foundation, Park Ridge, Illinois, 60068, the sum of $__________ [to be used by the (insert name of hospital or program) for its general purposes or according to a letter of intent previously agreed to by the Foundation and me]."

Residual Bequest – "I give the Advocate Charitable Foundation, Park Ridge, Illinois, 60068, _______ percent of the residue of my estate [to be used by the (insert name of hospital or program) for its general purposes or according to a letter of intent previously agreed to by the Foundation and me]."

Contingent Bequest – "In the event that ______________________ predeceases me, I give the Advocate Charitable Foundation, Park Ridge, Illinois, 60068, the sum of $___________(or, alternatively, __________ percent of the residue of my estate) [to be used by the (insert name of hospital or program) for its general purposes or according to a letter of intent previously agreed to by the Foundation and me]."

Recommended Provision for All Restricted Gifts

To ensure that Advocate Charitable Foundation will be able to make use of all bequests intended to support its mission and to address possible situations of future obsolescence, i.e. cases where a cure for a particular disease may be found, the Foundation encourages inclusion of the following:

"If, in the opinion of the Board of Directors of Advocate Charitable Foundations or their successors, the need for funds for the purpose described above no longer exists at some future date, the Directors or their successors are authorized to use these funds in the best interest of (name of hospital) for a purpose similar as practical to the donor's stated interest."
