You Can Become a Member of The Amicus Society!

How can I become a member?

To become a member, you can:

• Provide for one or more of Advocate’s hospitals or programs in your will or living trust
  
  
• Set up an income-producing planned gift – like a charitable remainder trust, a charitable gift annuity, a deferred-payment charitable gift annuity or a charitable lead trust – naming one or more of Advocate’s hospitals or programs as a beneficiary
  
  
• Arrange for one or more of Advocate’s hospitals or programs to have a remainder interest in your home or farm
  
  
• Make a gift of life insurance to Advocate
  
  
• Designate Advocate as a beneficiary of your retirement plan
  

Becoming a member is easy!

When you make any type of planned gift benefiting Advocate, just let us know. You will automatically be a member of the Amicus Society. All you need to do is tell us the type of planned gift you have arranged for us. You need not reveal the amount or give us copies of your legal papers. Of course, you can always choose to join and remain anonymous.

After you have made your planned gift to us, please fill out the Amicus Society Membership Form by clicking here. Then, please e-mail your completed form to donald.gottesman@advocatehealth.com. Thank you for your planned gift to Advocate.
